Room not as promised

We had specifically reserved a handicap accessible room as I was traveling with my mom. When we got there, they did not have an accessible room available, regardless of the reservation guaranteeing one. While they were able to offer us a bottom floor room, there was a curb to get into the room, and the shower couldn't be used by my mom because it wasn't an accessible shower. We also found a live cockroach on the bed skirt.
